Publish draft notes

Publish draft notes to the production tracking system. Only drafts matching a (user_email, version_id) pair in targets are considered; for each pair the most recently updated draft wins.

Per draft: a new draft creates a Note (authored as the draft owner) and records its ID; a draft already published and since edited updates that Note; one with no body but a version_status only updates the version's status; an unchanged published draft is skipped. Attachments are uploaded and cleared. If the version has a published transcript, the Note is linked to its newest transcript row (best effort; a missing row or field is logged and the note still publishes). Failures are counted in failed_count rather than failing the request, so always check the counts.
